Psychiatric advance directives are legal documents that allows people with mental illness to state their preferences for treatment in advance of a mental health crisis.

For Clients: for medication concerns such as reports of side effects or refill requests, call 440-204-4220
For Pharmacies: to leave messages requesting refills, diagnosis codes, clarification of orders, verbal requests for prior authorizations, etc., call 440-204-4220
A nurse checks the Medline daily. Staff will respond within 48-72 hours. We ask that only one request be made during that timeframe.
